Tesamorelin and Ipamorelin: What the Research Shows

Research reference · Updated October 2026 · ~8 min read

Tesamorelin and ipamorelin are discussed together because they raise growth hormone through different receptors. Tesamorelin is a stabilized analog of growth hormone-releasing hormone (GHRH). Ipamorelin is a selective agonist of the ghrelin receptor, GHS-R1a. Those two signals can amplify one GH pulse inside the pituitary. This page covers the mechanism, the human data for each peptide, and the missing combination trial. It is a research reference only, not a dosing guide and not for human use.

Two receptors, one pulse

GHRH binds its receptor, raises cyclic AMP, and drives GH secretion. Ghrelin, and synthetic ligands such as ipamorelin, bind GHS-R1a and raise intracellular calcium through phospholipase C. Studies of GHRH given with growth hormone-releasing peptides found a combined pulse larger than either signal alone. Tesamorelin supplies the GHRH side. Ipamorelin supplies the ghrelin-receptor side, with less cortisol, ACTH, and prolactin spillover than GHRP-2 or GHRP-6 in swine and rodent models (Raun et al., 1998).

That synergy is physiology, not an approved combination. No pivotal trial has tested tesamorelin plus ipamorelin. A stack note extrapolates from GHRH plus GHRP work plus separate data on each molecule. For a head-to-head rather than the pair, see tesamorelin versus ipamorelin.

What tesamorelin trials actually measured

Tesamorelin is the 44-amino-acid sequence of human GHRH with a trans-3-hexenoyl group on the N-terminus. The modification resists dipeptidyl peptidase cleavage better than native GHRH. Published figures put elimination half-life at about 26 to 38 minutes, with subcutaneous bioavailability at or below 4 percent. It is a short pulse, not a week-long depot. Effects on IGF-1 and visceral fat are summarized in what tesamorelin does.

In HIV-associated lipodystrophy, pivotal trials used 2 mg subcutaneously once daily. Visceral adipose tissue fell by roughly 15 percent from baseline over 26 weeks, IGF-1 rose, and the fat change reversed after withdrawal. FDA authorization covers only reduction of excess abdominal fat in adults with HIV and lipodystrophy, not recovery, general weight loss, or longevity research. Research-protocol summaries often cite 1 to 2 mg once daily outside that prescription setting. Those figures are research-reported, not dosing advice. The tesamorelin dosage page collects them.

What ipamorelin adds

Ipamorelin is the pentapeptide Aib-His-D-2-Nal-D-Phe-Lys-NH2. In the original animal work it released GH without a meaningful ACTH or cortisol rise at GH-releasing doses, and it did not drive appetite the way GHRP-6 does. It is not an approved medicine, and human programs never produced a labeled dose. Research-compound protocols commonly cite 100 to 300 mcg per subcutaneous injection, one to three times daily. That range is a literature convention, not a clinical recommendation. Selectivity and body-composition findings sit under ipamorelin peptide benefits.

Why researchers pair them

A GHRH analog sets pulse amplitude. A GHS-R agonist lowers the threshold for release and can still provoke GH when somatostatin tone is high. That is why short-acting CJC-1295, without Drug Affinity Complex, is commonly written up with ipamorelin. The CJC-1295 and ipamorelin dosage guide is the better-documented version of the pattern. Tesamorelin is sometimes swapped in as the GHRH half because of its dataset in one disease. That swap is not equivalence, and lipodystrophy trial results do not describe a multi-dose stack in other models. The CJC-1295 protocol page separates the DAC and non-DAC forms.

Research-reported figures

The table collects figures from trials or from research-compound protocols. None of these rows is a suggested stack, and the combination row is empty on purpose.

These numbers describe what has been published or repeated in laboratory protocols. They are not instructions to administer either peptide, alone or together.

No combined clinical dose exists. A chart that pairs tesamorelin milligrams with ipamorelin micrograms as one regimen is an extrapolation unless it cites a trial. Tested suppliers are listed on the supplier page.

Timing, reconstitution, and monitoring

Research protocols usually place a secretagogue exposure in a fasted window, because insulin and free fatty acids blunt GH release. Both peptides are typically lyophilized powders, so the volume that matches a stated amount depends on reconstitution. Use the peptide calculator before an in vitro or animal protocol is written down. Handling notes for ipamorelin are on the ipamorelin protocol page, and the protocol index covers storage for the wider set.

Clinical tesamorelin programs monitor IGF-1 and glucose. Labeling warns about glucose intolerance, and exposure is interrupted if IGF-1 stays above the age-adjusted range. A pair that drives the axis from two receptors would be expected to raise IGF-1 at least as far as tesamorelin alone. That expectation has not been measured in a controlled combination study.

Side effects and open questions

Tesamorelin trial reports include injection-site redness and itching, peripheral edema, joint pain, extremity pain, and diarrhea. Hypersensitivity has occurred. Ipamorelin's preclinical advantage is selectivity, not a blank safety file. Long-term human data are sparse, and fluid retention or glucose shifts remain plausible whenever GH and IGF-1 rise.

Combination-specific risk is unquantified. Additive IGF-1 elevation, overlapping injection-site reactions, and interactions with other research peptides have no controlled dataset. Community notes sometimes mention 8 to 12 week cycles. Those cycles are not backed by a tachyphylaxis study of this pair. In the lipodystrophy trials, tesamorelin dosing was continuous, and visceral fat returned after withdrawal.
